What Application Form Do I Use?
 
In order to ensure timely processing of your application, please carefully read the instructions on the appropriate form below to ensure you submit the required documentation and payment.
 
First-Time Applicant:

If you are a first-time applicant, or are unsure of your eligibility, complete and submit forms ED 170, ED 125 and/or ED 126.  Candidates seeking certification for: vocational education, adult education, school business administrator and/or school social work should complete and submit form ED 170.
 
First-Time Applicants From a Connecticut Educator Preparation Program:
 
If you are a first time applicant, complete and US mail ED 170A.
 
Renew Initial Educator Certificate:

If you currently hold an initial educator certificate and are applying to renew the initial educator certificate because you have not served under it for sufficient time to advance, use form ED 183 and ED 126.
 
Initial Educator Certificate to a Provisional Educator Certificate:

If you currently hold an initial educator certificate and are applying for a provisional educator certificate, complete and submit forms ED 170 and ED 126.
 
Provisional Educator Certificate to Professional Educator Certificate:

If you currently hold a provisional educator certificate and are applying to advance to a professional educator certificate, complete and submit forms ED 170 and ED 126.
 
Renew Professional Educator Certificate:

If you currently hold a professional educator certificate and are applying to renew/continue the professional educator certificate, use form ED 179
 
Additional Teaching Endorsement:

To request a cross endorsement, use form ED 170.
 
Duplicate Certificate:

To request a duplicate certificate, including as a result of a name change, use form ED 187. Note: you may update your name and print out an unofficial copy of your certificate from your account on the CECS.
 
Coaching Permits:

To apply for a Temporary Emergency Coaching Permit, use form ED 186.
To apply for a Five-Year Renewable Coaching Permit, use form ED 185.
