EXEMPLARY PRACTICES NOMINATION FORMS

The Connecticut State Department of Education (CSDE) has established a CSDE-Institutes of Higher Education (IHE)-Local Education Agency (LEA) workgroup to advise the department on strategies to integrate the four foundational CALI modules (School and Instructional Data Teams, Getting Ready for the Next Generation of Assessments, Using Differentiated Instruction to Implement the Common Core State Standards, and Improving School  Climate) into course syllabi and field experiences.

Among the workgroups recommendations was the development of a webpage that would be a "one stop" shop for higher education faculty to access pertinent information from the CSDE website and the development of a Speakers Bureau that higher education could use to invite practitioners to guest speak in their classes. We are looking to identify schools and districts that exemplify best practices in the four areas (data teams, assessment, differentiation of instruction, and positive school climate) for inclusion on the CSDE website and in the Higher Education Speakers Bureau. If you would like to nominate your school and/or district for an exemplary practice designation, please fill out the attached forms and submit to by Friday, June 15th. Forms can be submitted to me by email: iris.white@ct.gov, by fax:  860-713-7035, or by mail:  165 Capitol Ave, Room 227, Hartford, CT 06106.  All nominations will be reviewed by the CSDE-IHE-LEA workgroup this summer.
